How do I apply?

Applying for GraceWell is easy. Let's get started:

Before applying for membership, you should familiarize yourself with the GraceWell Guidelines.

Please have all of the information below available when you're ready to apply:

  • Personal data, including name, date of birth, Social Security Number, Government-issued ID number (e.g., Driver's License).
  • Contact information, including email address and phone number.
  • Banking information, including bank routing number and account number to pay your application fee and first Monthly Share payment.
